Better Health Partnership's 2025 Learning Collaborative brings experts together to learn how to better engage the community in efforts designed to improve health equity, build trustworthiness in collaboration, and leverage EMR and other best practices for better population health management. We will also celebrate a collaborative champion in the region and provide opportunities for attendees to connect with fellow colleagues and be inspired to make a difference in the communities you serve.

1. Identify opportunities to connect children without a medical home for pediatric care and lead testing.
2. Identify common sources and risk factors for childhood lead exposure and strategies for prevention.
3. Describe effective strategies to address key barriers to transitions of care.
4. Describe how community health centers face barriers in scaling best practices in clinical care.
5. List key barriers to perinatal care in Cuyahoga County.
6. Describe Food as Medicine programs and how they can be applied in different medical practice settings.
7. Describe the National CLAS Standards and apply strategies to promote culturally and linguistically appropriate services, including recognizing bias and enhancing effective communication in healthcare settings.
8. Identify ways to align community partners, funding, and data to protect health
