Overview
The ECHO Clinical Care and Case Management for Transition-Age Individuals with Autism series is designed to equip care coordinators, case managers, and other professionals with the knowledge, skills, and connections needed to support a successful transition from pediatric to adult services for individuals with Autism Spectrum Disorder (ASD). Using the ECHO model’s “all teach, all learn” approach, participants will engage in 8 sessions that combine brief expert presentations, real-world case discussions, and peer-to-peer learning. This series will address the complexities of transition planning by focusing on person-centered approaches, continuity of care, and navigation of adult service systems. Participants will explore topics such as healthcare and behavioral health access, vocational training, housing, life skills development, and community integration. Legal rights, advocacy, and self-determination will be emphasized to empower both professionals and the individuals they serve. Sessions will also integrate the perspectives of parents and caregivers, offering valuable insights into family priorities, advocacy experiences, and the practical impact of support systems. By connecting professionals across disciplines and regions, this ECHO will create a supportive learning network where participants can share expertise, gain strategies to address service gaps, and build confidence in guiding individuals with ASD toward successful adult lives.

Accreditation
The MetroHealth System is accredited by the Ohio State Medical Association to provide continuing medical education for physicians.

Cancellation Policy

Cancellations received in writing no less than 20 days before the course will be refunded, less a 20% administrative fee. No refunds will be made on cancellations received after that date. Please send cancellation requests to [email protected].
